To order dish TV service online you need the above information.
To start you will need the form listed in step 5.

Fill in the first and last name of the person that will appear on the bill as well as the address where you want satellite tv.
If you are married or have roommates, pick just one name.

You also need a phone number. It is fine if the number is a cell phone.

Lastly you will need to have an email address.  If for some reason you do not have an email address you can use a friend's or family member's.
